Physical Therapy abuse and over usage. A doctor told Facility X they could see his patients for PT only. Someone wrote up a false order to have his patient have PT and a Nurse so that they could make more money from Medicare.

I work in the XYZ Department at the XYZ Facility weekdays. Exaggerating insurance claims and upcoding happens all the time.
